Form-Fill Option
You must have Adobe
4.0 or higher.
Click on the form
you want to use and it will open in Acrobat.
Select the hand tool
Position the hand
inside a form field (the hand will have a pointed
finger when you enter a form field), and click.The
I-beam pointer allows you to type text. The hand tool
will also allow you to select the check boxes.
If you prefer you
may also use the Tab key to go to the next form field
or press Shift+Tab to the go to the previous form
field.
When you have
finished filling out the form, print the document and
bring it with you to your first appointment.
Warning: If you do
not have Adobe Acrobat
Professional you will not be able to save the
form.
