Request Your Medical Records:

Step 1: Download and complete the Authorization for Release of Health Information form.

Step 2:  Submit completed authorization form by:

Monday – Friday, 8 AM - 4 PM | Closed for Lunch 12-12:30 PM

Step 3: Please allow up to 14 business days for records to be processed.

Step 4: Pick Up and Pay for Records 

Fee Schedule for Records Processing:

Patient, Attorney and Copy Service Fees:  

$0.25 per printed page + postage if mailed. 

$6.50 flat rate if faxed (limit of 25 pages)

Subpoena Fees:

$15.00, plus $0.10 per printed page. 

Note: Subpoenas must either be served in person or by mail.

No subpoenas will be accepted by fax.
