NEW PATIENT GUIDE

Appointments:

  • Please arrive 20 minutes early to check in and pay your applicable co-pay/co-insurance.

  • Upon scheduling your appointment you will receive an onboarding form through text, please complete this digital form within 48 hours to confirm your appointment.

  • Occasionally emergencies arise that may cause the delay of your appointment. We will make every effort to keep such delays to a minimum and will keep you informed of delays that occur.

  • If you are unable to keep your scheduled appointment, please call our office at least 24 hours in advance. Also, if you know that you will be late, please call. This courtesy allows us to accommodate other patients. 

Bring the following to your appointment:

- A list of all the medications you are currently taking.

- Any and all X-rays, medical records, referral forms, and all diagnostic testing results - that you have.

- Valid Photo ID

- Current insurance card(s).

- Co-Pay/Co-Insurance payments, collected at check-in

If your visit pertains to a worker's compensation injury please also complete the worker's compensation form in the packet. More information on worker’s compensation can be found here.
