As required by the California Business and Professions Code Section 654.2, this notice is to inform you that the above Arrowhead Orthopaedics Physicians have ownership interest in some or all of the following entities: Advanced Ambulatory Surgery Center, Alliance Surgical Distributors, Arrowhead Orthopaedics, Arrowhead Orthopaedics Physical/Occupational Therapy, AxS Healthcare, Orthopedic Professional Associates, Prior Level Home Care Physical Therapy, Renovis Surgical Technologies, San Antonio Advanced Ambulatory Surgical Center, and Seven Oaks Surgery Center. Your Arrowhead Orthopaedics physician may recommend that you receive services or implants from these ancillary organizations. Please be advised, however, that you are free to obtain implants or instrumentation, hospital services, therapy services, and imaging services from any provider of your choosing.

These physicians maintain this ownership interest in order to ensure their ability to deliver the highest quality orthopedic care and in a timely and convenient manner.
